Job Description
Kaiser Permanente seeks a Medical Social Worker III (LCSW) in Anaheim to deliver advanced clinical social work services in an on-site healthcare setting. In this per diem role, you will complete comprehensive psychosocial assessments, provide counseling, crisis intervention, and short-term therapy, and develop safe, effective discharge and care plans. You will collaborate closely with physicians, nurses, and care teams, coordinate referrals to community resources, and advocate for patient access and rights while documenting all care in the EHR. This position supports quality, continuity of care, and optimal patient outcomes.
