The Licensed Clinical Social Worker (LCSW) provides behavioral health services within an integrated healthcare setting, including assessment, diagnosis, treatment planning, psychotherapy, crisis intervention, and care coordination. Working collaboratively with medical providers, behavioral health professionals, and community partners, the LCSW delivers high-quality, evidence-based care that supports patients' overall health and well-being. This role promotes a patient-centered approach to care by addressing behavioral health needs and connecting individuals with appropriate resources and support services. The LCSW also contributes to interdisciplinary care planning, quality improvement initiatives, and program development while helping advance access to culturally responsive behavioral health services. This is a Part-Time position. Applicant must be able to work on-site Tuesday , Wednesday , and Thursday .DUTIES AND RESPONSIBILITIES
Provide Clinical Social Work Services including psychosocial assessments, diagnosis, treatment planning, and psychotherapy with children, adolescents, adults, parents, and families. Opportunity to supervise and train interns and unlicensed staff, as needed. Maintain quality assurance of the behavioral health program. Work with a multidisciplinary team to ensure the overall health and well-being of the patient. Conduct intake and assessment of patients' available resources and needs. Formulate assessments and direct treatment accordingly. Develop an individual-based and/or group-based treatment plan, including medical, psychological, financial, and other life issues. Maintain confidentiality at all times. Participation in Via Care Community Health Center's Continuing Quality Improvement (QI) program. Contribute to program planning, evaluation, report preparation, and development of articles. Performs other tasks as required or as assigned. Maintain a positive working relationship with staff. Other duties as designated by the Medical Director or Executive Director Must be fluently bilingual in English and Spanish. MSW from graduate school for Social Work Licensed in the State of California by the Board of Behavioral Science Examiners Demonstrated expertise/skills in working with the target population Two (2) years of experience in a Child/Family setting (including internships) Able to handle emergency or crisis situationsSPECIAL REQUIREMENTS
Valid California Driver License and automobile insurance coverage Must be able to travel to off-site and satellite locations Subject to a criminal background check prior to employment TB clearance, to be renewed every year.
