Job Description
Jackson Therapy Partners is seeking a Travel Licensed Clinical Social Worker (LCSW) to provide compassionate, patient-centered behavioral health support across diverse healthcare settings nationwide. In this travel role, you'll complete psychosocial assessments, deliver individual and group counseling, coordinate discharge and community resources, and collaborate with interdisciplinary teams to support optimal patient outcomes. Responsibilities Provide comprehensive biopsychosocial assessments for patients across diverse healthcare settings as a traveling LCSW. Develop and implement individualized treatment and discharge plans in collaboration with the interdisciplinary care team. Deliver counseling, crisis intervention, and brief therapeutic support to patients and families facing acute and chronic health conditions. Identify and address barriers to safe discharge, treatment adherence, and psychosocial stability, including financial, housing, and transportation issues. Coordinate referrals and link patients and families to appropriate community resources and support services. Document assessments, interventions, and care plans accurately and timely in the electronic health record, following facility and regulatory standards. Collaborate closely with physicians, nurses, therapists, and case managers to ensure integrated, patient-centered care. Adapt quickly to new facilities, policies, and workflows with each travel assignment while maintaining high professional and ethical standards. Educate patients and families on coping strategies, community resources, and care options, including advanced care planning when appropriate. Maintain licensure, certifications, and compliance with all state, facility, and agency requirements while traveling between assignments.
