Job Description
Job Summary We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. The ideal candidate will provide essential support to patients and their families, assisting them in navigating the complexities of healthcare systems while addressing emotional, social, and financial challenges. This role requires a strong understanding of medical terminology and the ability to work collaboratively with healthcare professionals to ensure holistic patient care. Duties Conduct assessments to identify patients' social, emotional, and environmental needs. Develop and implement individualized care plans in collaboration with multidisciplinary teams. Provide counseling services, including child family counseling and behavioral therapy, to patients and their families. Assist patients in accessing community resources and services related to dementia care, developmental disabilities, and behavioral health. Educate patients and families about medical conditions, treatment options, and available support systems. Maintain accurate documentation of patient interactions and progress in compliance with clinical research protocols. Advocate for patients' rights and needs within the healthcare system. Participate in case conferences and team meetings to discuss patient progress and adjust care plans as necessary. Qualifications Master's degree in Social Work (MSW) from an accredited institution. Experience in child family counseling, dementia care, or working with individuals with developmental disabilities is preferred. Knowledge of medical terminology and coding practices is advantageous. Proficiency in psychotherapy techniques and behavioral health interventions. Strong communication skills with the ability to empathize with patients from diverse backgrounds. Ability to work effectively within a clinical setting while maintaining confidentiality.
