Hospice MSW- Inland Empire
Job Description
Job Summary:
We are seeking a dedicated and compassionate Hospice Medical Social Worker (MSW) to serve patients and their families within the Inland Empire region. The ideal candidate will possess a strong background in social work practice standards, hospice care, and end-of-life support, with a commitment to delivering comprehensive psychosocial assessments and interventions. As a vital member of our interdisciplinary team, you will coordinate community resources, provide clinical counseling, and ensure patients receive personalized, culturally sensitive care that enhances their quality of life during challenging times.
Responsibilities:
Conduct thorough psychosocial assessments to evaluate patients' emotional, social, and spiritual needs in accordance with hospice care standards. Develop individualized care plans that incorporate patient preferences, family dynamics, and available community resources. Provide clinical counseling services including grief support, behavioral therapy, and end-of-life discussions to patients and families. Coordinate with healthcare providers to facilitate discharge planning, medical documentation, and continuity of care across inpatient and home settings. Assist with crisis intervention and behavioral health management for patients experiencing acute emotional distress or mental health challenges such as PTSD or depression. Maintain accurate medical records and documentation in compliance with HIPAA regulations and clinical ethical guidelines. Collaborate effectively within an interprofessional team to optimize patient outcomes through case management, motivational interviewing, and therapeutic interventions. Support hospice care initiatives by participating in clinical research efforts related to palliative medicine and geriatric patient populations.
Experience:
Proven experience working with individuals from diverse cultural backgrounds in hospice or palliative care settings. Demonstrated expertise in medical social work practice standards, including intake procedures, discharge planning, and medical documentation. Familiarity with medical terminology, ICD coding, Medicare requirements, and EMR systems used in healthcare environments. Prior inpatient or nursing home experience with a focus on geriatrics, dementia care, Alzheimer's disease, or developmental disabilities is highly desirable. Experience providing behavioral health services such as individual/group counseling, crisis management, or cognitive behavioral therapy (CBT). Knowledge of hospice & palliative medicine principles along with skills in end-of-life care planning and family support strategies. Ability to work effectively with patients experiencing complex conditions such as PTSD or autism spectrum disorders while maintaining compliance with social work practice standards.
Qualifications :
Master's Degree from an accredited school of social work or related field Prior Hospice experience-1 year (Required) State licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) as required Must possess and maintain valid CPR certification while employed in a clinical role Must be a licensed driver who can travel to all business locations
Pay:
From $40
•$45 per hour DOE $85
•90 per visit rate
