Per visit rates + mileage •rates are determined by experience• Job Summary We are seeking a compassionate and dedicated Hospice Medical Social Worker to join our interdisciplinary care team. In this vital role, you will provide emotional support, counseling, and case management services to patients facing life-limiting illnesses and their families. Your expertise will help improve quality of life, facilitate effective communication, and ensure comprehensive psychosocial care in a hospice setting. This position offers an opportunity to make a meaningful difference by guiding patients and families through complex emotional and practical challenges associated with end-of-life care. Duties Conduct thorough patient assessments to identify psychosocial needs, including mental health status, family dynamics, and social support systems Develop individualized care plans that incorporate behavioral health strategies, counseling techniques, and community resources Provide clinical counseling such as grief support, behavioral therapy, motivational interviewing, and crisis intervention to patients and family members Facilitate group therapy sessions focused on coping skills, bereavement support, or educational topics related to hospice care Collaborate with healthcare teams to coordinate services across home & community care settings, assisted living facilities, nursing homes, and inpatient units Maintain accurate medical documentation in compliance with HIPAA regulations and medical record standards using EMR (Electronic Medical Records) systems Assist with intake procedures, discharge planning, and follow-up assessments to ensure continuity of care Requirements Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program; licensure or certification as required by state law Proven experience in hospice care, behavioral health, or medical social work within inpatient, outpatient, or home & community settings Familiarity with medical records management and EMR systems( Kinnser/Wellsky ); understanding of HIPAA compliance standards Experience working with diverse populations including children, seniors, individuals with disabilities or developmental disabilities, dementia or Alzheimer's disease patients Excellent communication skills for patient care planning, family counseling, and interdisciplinary collaboration Ability to handle crisis situations effectively while demonstrating empathy and professionalism Join us in providing exceptional psychosocial support that enhances the dignity and comfort of those we serve.
