Position Summary As a Licensed Clinical Social Worker (LCSW) with Impact Health PC in New Haven, CT , you will provide psychotherapy and behavioral health services to residents within skilled nursing facilities. You will manage an assigned caseload, conduct assessments, provide individual therapy, and collaborate with psychiatric providers, facility staff, and interdisciplinary care teams. Key Responsibilities Provide individual psychotherapy and person-centered behavioral health care. Conduct clinical assessments and identify residents' behavioral health needs. Manage an assigned caseload within skilled nursing facilities. Develop and support individualized treatment plans. Collaborate with psychiatric providers, medical staff, facility teams, and other members of the interdisciplinary care team. Educate and support residents, families, and facility staff as appropriate. Complete timely and accurate clinical documentation. Maintain compliance with facility, state, and federal regulations.
