Job Description
Job Summary The Medical Social Worker will be responsible for providing support and counseling to patients dealing with various medical conditions. They will collaborate with healthcare professionals to ensure the well-being of patients and assist in creating effective treatment plans. Duties Conduct patient assessments to identify emotional, social, and psychological needs Develop and implement care plans tailored to each patient's specific situation Provide counseling and support to patients and their families Coordinate with healthcare team members to optimize patient care Advocate for patients' rights and access to necessary resources Maintain accurate and confidential patient records Offer guidance on community resources and support services Qualifications Bachelor's or Master's degree in Social Work or related field Experience in cognitive behavioral therapy, case management, and mental health counseling Familiarity with working with individuals with developmental disabilities or special education needs Knowledge of inpatient settings and applied behavior analysis is a plus Strong interpersonal skills and the ability to work effectively in a multidisciplinary team Excellent communication skills for interacting with patients, families, and healthcare professionals
