Job Description

Overview We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our team. This position at the Council on Aging Martin County would provide counseling, consultation and guidance for families learning to cope with a loved one's dementia diagnosis and other aging conditions. The ideal candidate will possess a strong background in mental health counseling and social work, with a focus on providing high-quality care to individuals and families. This role involves working collaboratively with clients to assess their needs, develop treatment plans, and facilitate therapeutic interventions. The LCSW will play a vital role in promoting mental well-being and supporting clients through various avenues, while facilitating caregiver support groups and providing education. Responsibilities Conduct comprehensive assessments and diagnostic evaluations of clients. Develop and implement individualized treatment plans based on client needs. Provide psychotherapy, including individual, group, and family counseling. Utilize motivational interviewing techniques to encourage client engagement. Deliver crisis intervention services as needed to ensure client safety. Maintain accurate medical documentation and adhere to HIPAA regulations. Collaborate with multidisciplinary teams to coordinate patient care. Engage in case management activities to support clients in accessing resources. Participate in clinical research and data collection for program evaluation. Provide education and support for families dealing with developmental disabilities, PTSD, addiction issues, and other mental health concerns. Skills The successful candidate will demonstrate proficiency in the following areas: Psychotherapy techniques including cognitive behavioral therapy (CBT) and psychodynamic therapy. Experience with geriatric & family counseling, and working with individuals with aging conditions. Knowledge of medical terminology, EMR systems, CPT coding, ICD-9, and ICD-10 coding practices. Strong skills in behavioral therapy, crisis management, and patient assessment. Familiarity with home & community care experience as well as geriatric care. Ability to conduct clinical research and effectively document medical records. Experience in addiction counseling and providing support for PTSD care. Understanding of special education resources available for children with disabilities. Excellent communication skills for effective patient interaction and group therapy facilitation.
