We are seeking a compassionate, experienced, and dedicated Licensed Clinical Social Worker (LCSW) for an exciting hospital-based travel assignment in Lawrenceville, Georgia. Key Responsibilities Conduct comprehensive psychosocial assessments for hospitalized patients. Develop individualized care plans and coordinate discharge planning. Provide crisis intervention, counseling, and emotional support to patients and families. Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure continuity of care. Connect patients with appropriate community resources and post-discharge services. Advocate for patients while maintaining compliance with hospital policies and ethical standards. Accurately document patient assessments, interventions, and care plans in the electronic medical record. Minimum Qualifications Active Georgia LCSW License (Required) AHA BLS Certification (Required) Minimum of 2 years of recent hospital-based social work experience Strong communication, assessment, and care coordination skills Ability to thrive in a fast-paced acute care environment
