Full-time and part-time positions available

Experience Level:

0-3 years as a social worker

Responsibilities:

Provide direct clinical services to patients, including assessment, diagnosis, treatment planning, and counseling. Conduct psychosocial assessments and develop individualized care plans. Collaborate with interdisciplinary teams to ensure comprehensive patient care. Travel to patients' homes for assessments, follow-up visits, and crisis intervention. Maintain an active LCSW license in the state of Illinois. Adhere to ethical and legal standards in social work practice. Participate in case conferences and team meetings. Document patient interactions accurately and timely. Assist patients in accessing community resources and support services. Provide crisis intervention and support during emergencies. Engage in ongoing professional development and training. Psych therapy patient evaluation

Qualifications:

THIS IS A PAY PER VISIT JOB.

Master's degree in Social Work (MSW) from an accredited program. Active LCSW license in the state of Illinois. Strong communication and interpersonal skills. Ability to work independently and as part of a team. Compassionate, empathetic, and culturally sensitive. Proficient in electronic health records (EHR) systems.

HOSPICE MD, AXXCESS

Knowledge of relevant laws, regulations, and ethical guidelines. Experience working with diverse populations and addressing mental health needs. Willingness to travel to patients' residences.

Compensation:

Negotiable based on experience and skill level.
