Medical Social Worker Job Description:
Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required. Travels to patient's homes and facilities to conduct visits, discuss hospice philosophy and assist with recruitment of clientele within servicing county areas. Carries out psychosocial evaluations and plans interventions based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Maintains timely patient care records including observations, interventions and changes in patient conditions. Provides information and referral services for patients and families/caregivers regarding practical and environmental needs. Serves as liaison between patients and families/caregivers and community agencies. Maintains collaborative relationships with organization personnel to support patient care. Participates in the development of the individualized plan of care and attends regularly scheduled interdisciplinary group meetings. Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues. Actively participates in quality assessment performance improvement teams and activities. Other duties as delegated by the Hospice Manager. Job Requirements Flexible to travel throughout coverage areas. Must be able to conduct in-home & nursing facility visits. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order. Possesses and maintains current CPR Certification. Demonstrates appropriate and professional behavior in the workplace. Knowledge of community resources or the desire/ability to obtain this knowledge a plus. Knowledge of key psychosocial issues prevalent with terminally ill patients and their family/caregivers, including the knowledge of effective psychosocial interventions. Demonstrates strong verbal, written communication, and organization skills. Capacity to work well with others, to be an effective patient and family advocate- working with families, the IDG team, and within associated community agencies. Ability to exercise independent professional judgement in complex situations. Master's Degree in Social Work or related field required. Qualifications Master of Social Work from a school accredited by the Council on Social Work Education or related field. At least one year social work experience in a health care setting. Experience of social work with the terminally ill and their families, preferred. Spanish speaking, preferred.
