The Crisis Center is seeking a dedicated and reliable Full-Time Family Preservation Case Manager to join our Community-Based Services Department. This position provides intensive, home-based services to families referred by the Indiana Department of Child Services (DCS) to safely prevent out-of-home placement and preserve the family unit. POSITION RESPONSIBILITIES ESSENTIAL FUNCTIONS
Provide comprehensive, home-based family preservation services in compliance with Indiana DCS standards. Complete initial assessments and develop individualized service plans. Conduct at least one mandatory weekly safety home visit. Provide parent education, casework services, safety planning, crisis intervention, and transportation as needed. Deliver 24/7 crisis response support as required by the service model. Maintain accurate, timely documentation and case records. Attend Child and Family Team Meetings (CFTM) and court hearings as requested. Collaborate with DCS and community partners to ensure child safety and family stability. Participate in supervision, staff meetings, and required trainings. Expectations Maintain professional boundaries and ethical standards. Deliver culturally responsive, family-centered services. Meet DCS contract benchmarks and documentation deadlines. Work flexible hours, including evenings and weekends as needed. Manage multiple cases effectively and independently.
QUALIFICATIONS
Master's or Doctorate degree in Social Work, Psychology, Marriage & Family Therapy, or related human services field. Preferred license includes LSW, LCSW, LMHC, LPC, or eligibility to obtain license in the state of Indiana. Minimum 2+ years of experience in child welfare, crisis intervention, behavioral health, or community-based services. Valid driver's license and reliable vehicle. Must pass a Tuberculosis (TB) test. Ability to pass National and State Criminal History Check. Fingerprint-based Background Check Child Protective Services (CPS) Background Check National Sex Offender Registry Check Local Criminal Court Records Check Work Environment This position is based at Crisis Center, Inc. with regular travel across Lake and Porter Counties. Morning, evening or weekend work may be required to meet family or program needs.
