Home - Fortis About the Role We are seeking a compassionate and reliable Home Health & Hospice Licensed Clinical Social Worker (LCSW) to support patients and families served by Select Home Health and Hospice. In this role, you will provide professional medical social services in the home, addressing social, emotional, and psychosocial needs while supporting patient dignity, comfort, and quality of life in accordance with the plan of care. Responsibilities Provide medical social services to hospice patients and families in the home setting. Assess and address social, emotional, and psychosocial needs related to illness and end-of-life care. Collaborate with the interdisciplinary team to develop and support the patient's plan of care. Counsel patients and families to support coping, adjustment, and decision-making. Communicate changes in patient condition to the supervisor and physician as needed. Connect patients and families with appropriate community and social service resources. Participate in care coordination, discharge planning, and team meetings.
