Job Description
Oak Street Health seeks a Medical Social Worker Float for the Louisville market to support Medicare patients in underserved communities. You will provide psychosocial assessments, care planning, brief counseling, and crisis intervention while addressing social determinants of health. In this floating role, you'll cover multiple clinics, coordinating community resources, benefits, and transitions of care.
