As a full time Medical Social Worker you will be an integral part of the care coordination team, working in collaboration with RN case management, the clinical team, and internal/external collaterals. As a member of our interdisciplinary team you will be responsible for performing psychosocial assessments, providing supportive counseling and care coordination with respect to discharge planning and service coordination for adult, geriatric, pediatric, and med/surg populations.

What You'll Bring:
  • A Master's degree in Social Work (MSW)
  • A minimum of one year related experience in a hospital environment and/or with transferable skills that apply to a fast-paced and high-urgency setting
  • Ability to triage crises and collaborate/coordinate with collateral agencies
  • Experience with protective services, child welfare, long term care, crisis intervention, chronic illness, hospice care, substance use disorders and psychiatric illnesses a plus-New graduates are encouraged to apply.
Licensure requirements can be discussed and obtained with an agreed upon timeline.
Schedule:
40 hours day shift, weekend and holiday coverage will be required, schedules are given months in advance.
Pay Range:
$72,800.00 USD - $97,989.00 USDThe pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
