Requirements
•BLS (AHA)
Licensures Accepted:
LMHC/LCSW/LICSW
in MA.
2+ years of Social Worker experience.
Job Description Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
Formulates biopsychosocial assessment, disposition, and treatment plans.
Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
Performs other duties as assigned
