As a member of the OBGYN care team, this individual provides clinical individual and group social work and facilitates access to resources and services to work toward continuous quality improvement and cultural competence of service delivery Receives referrals from OB providers when social work needs are identified Provides psychosocial assessment and interventions based on identified needs with mutually agreed upon goals with the patient Completes timely documentation in the patient's medical record reflective of assessment and interventions provided May provide brief counseling to patients when short-term needs are indicated Initiates referrals to community agencies for mental health issues and referrals for outpatient counseling Collaborates with OB clinical team when high risk issues are identified Receives referrals for substance use as identified through the substance use screening tool and connects patients with appropriate community resources Adheres to mandated reporting Establishes and runs peri-natal and postpartum support groups Maintains tracking and reporting systems to capture the peri-natal and postpartum support group participation Educates and advocates on behalf of patients, families, and or significant others Maintains high visibility in the community; makes presentations before hospital committees, community agencies, associations, media, physician groups, etc. as appropriate Serves as a liaison with key community agencies; provides connections for Signature Healthcare patients to community resources as appropriate Interacts with hospital and ambulatory departments to facilitate the operation and promotion of the program- including Maternity, Pediatrics (impatient and ambulatory) OBGYN department, primary care, and others May organize tours of Labor and Delivery floor at SHBH for pregnant women. Coordinates follow up care for mother, baby, and family post-delivery. Responds to requests for face and telephonic interpreter services to assist the patients, associates, and physicians associated with the OB Patient Navigation program Utilizes cultural competency knowledge as a cultural broker, informing providers and patients of relevant cultural issues and assisting providers in the delivery of culturally competent patient care Collaborates with hospital/community health care providers to determine the care and needs of prenatal women in the hospital and community Evaluates program components for effectiveness and efficacy through established Lean and Quality Improvement processes. Provides suggestions for improvement of patient satisfaction, clinical quality, and financial effectiveness. Participates in appropriate training, workshops, and community meetings to ensure continued development and compliance. Knowledge of area community resources Ability to prioritize multiple tasks, problem-solve and demonstrate initiative Excellent communication and critical thinking skills Respect for confidentiality of others Master's Degree in Social Work; LICSW required, 2-5 years healthcare experience preferred; Bi-lingual preferred; maternal mental health experience preferred. Experience with computer systems including web-based applications and some Microsoft Office applications, such as Outlook, Word, Excel, PowerPoint, or Access;
