- Medical Social Worker Full-Time | Direct Hire | $73,000-$85,000/year | Day Shift A hospital in Annapolis, MD is seeking an experienced Medical Social Worker to join its team. This is a full-time, onsite, direct-hire opportunity offering a Monday-Friday schedule with one weekend per month.

Position Highlights Salary:

$73,000-$85,000 per year, based on experience Full-time, day shift 8-hour shifts Monday-Friday + 1 weekend per month Onsite hospital position Direct Hire Collaborative multidisciplinary environment Qualifications MD LMSW or higher Inpatient hospital Social Work experience Discharge planning and/or case management experience required EPIC experience Strong communication, assessment, and care coordination skills Responsibilities Conduct early identification, high-risk screening, and readmission screening Provide counseling and support related to adjustment to illness Complete safety assessments Develop and coordinate discharge plans Identify obstacles and barriers to safe and timely discharge Coordinate with patients, families, and the multidisciplinary care team Connect patients with appropriate resources and services Collaborate with physicians, nurses, case managers, and other members of the healthcare team Interested?
