A Healthcare Social Worker provides individuals and families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.

Job Responsibilities:
The Therapist provides individual, family, and group counseling in our OMHC program. Services are provided in office, in contracted schools and virtually. The position requires completion of intakes and psychosocial assessments (e.g., screening for mental health and substance use concerns and mental status). The position also requires assessment of needs to include identification of current mental health, social, and emotional needs, making initial recommendations, in-house and outside referrals. In addition, the therapist is required to write appropriate summary reports, maintain appropriate documentation, and attend a weekly team meeting.
