Community Care Facilities and Homes What You'll Do:
Assesses and identify the client's holistic health care needs/concerns as part of a family and community. Assesses social/emotional factors that are related to the patient's illness, need for care, response to treatment, and adjustment to care. Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols. Implement patient care plan activities based on treatment goals towards independent living for the patient/family. Serve as resource for staff education & training, participate in case conferences as needed. Per Diem LCSW to support
Palliative Care, Hospice, and Home Health What You'll Bring:
Master's degree from a school of social work accredited by the council on social work education. Maine Board of Social Work Licensure- MSW or LCSW required Current CPR card required. One or more years of experience as a master's level social worker in a health care setting. A valid Drivers License and reliable car with insurance.
