A Healthcare Social Worker provides individuals and families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.
Michigan Community VNA is looking for a Full-Time LMSW (Social Worker) for our homecare and hospice patients in Southeast Michigan area (Livonia, Novi, Plymouth, Northville, West Bloomfield, Canton, etc.). A Social Worker who supports patients and their families emotionally, socially, and practically during end-of-life care. The role is part of an interdisciplinary hospice team that may include nurses, physicians, chaplains, aides, and counselors.
Key responsibilities:
Assess the patient's and family's psychosocial, emotional, and financial needs. Provide emotional support and counseling related to terminal illness, grief, dying, and family concerns. Help patients and families cope with changes, stress, loss, and end-of-life decisions. Participate in hospice care planning and interdisciplinary team meetings. Assist with advance-care planning, end-of-life resources, and other appropriate community services. Connect families with resources such as financial assistance, transportation, food, housing, caregiving support, or bereavement services. Advocate for the patient's wishes, dignity, comfort, and quality of life. Provide crisis intervention when needed. Help families understand hospice services and what to expect during the dying process. Provide or coordinate bereavement support before and after the patient's death. Maintain accurate clinical documentation and follow hospice and regulatory requirements. Make home, facility, hospital, or other patient visits depending on the hospice program. Typical qualifications Most hospice social-worker positions require: A state social-work license , LMSW . Experience in healthcare, hospice, palliative care, geriatrics, mental health, or counseling is often preferred. Strong communication, assessment, crisis-intervention, and documentation skills. Ability to work compassionately with patients and families from diverse backgrounds. Comfort discussing death, dying, grief, and difficult family situations.
