Join our compassionate team at Trinity Health St. Mary's Hospital, where your skills in clinical social work will make a meaningful difference in the lives of our patients and their families. In this role, you will conduct psychosocial assessments, provide brief solution-focused counseling, and develop collaborative treatment goals. Your efforts will help patients overcome barriers, access community resources, and ensure continuity of care across inpatient and outpatient settings.
What You'll Do:
Conduct comprehensive psychosocial assessments for patients and families. Develop treatment plans that address emotional, social, and resource-related needs. Provide brief, solution-focused counseling and facilitate discharge or care planning. Advocate for patients and coordinate referrals to community-based services. Collaborate effectively with interdisciplinary care teams. Document all assessments, plans, interventions, and outcomes clearly and promptly. Meet departmental productivity expectations and performance standards.
What You'll Bring:
Education:
Master's Degree in Social Work or a related field from an accredited institution.
Licensure:
Must be eligible for licensure as aLimited Licensed Master Social Worker (LLMSW) orLicensed Master Social Worker (LMSW) by the Michigan Board of Social Work.
Experience:
Minimum of 2 years of social work experience in an acute inpatient setting.

Strong psychosocial assessment skills and clinical judgment. Ability to navigate complex family dynamics and high-acuity situations. Excellent crisis support and problem-solving abilities. Outstanding interpersonal and empathetic communication skills. Proficiency in engaging patients and care teams in effective treatment planning.
