About the Role Provide compassionate, clinical psychosocial care that helps patients and families navigate the emotional, social, and practical challenges of end-of-life illness. As a Hospice Medical Social Worker, you'll support individuals facing terminal diagnoses by offering counseling, guidance, and resources that promote dignity, clarity, and emotional well-being during one of life's most difficult transitions. This role is ideal for a licensed clinical social worker who is passionate about practicing the full scope of hospice social work in a deeply meaningful setting. You'll complete comprehensive psychosocial assessments, provide crisis intervention and counseling, and support advance care planning conversations that help families make informed, values-based decisions. Working as a key member of the interdisciplinary team, you'll collaborate closely with nurses, physicians, and other hospice professionals to ensure emotional, cultural, and practical needs are fully integrated into each patient's plan of care. Your work will directly support both patients and families while also strengthening the care experience across the entire hospice journey.

Lansing and surrounding This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes Hours Monday to Friday, 8 a.m. to 5 p.m. Required Qualification Master's or doctoral degree from a CSWE-accredited school of social work Current State License as a Master's Social Worker (MSW/LMSW/LCSW) Minimum 1 year of experience as a licensed Master's Social Worker Reliable transportation, valid driver's license, and valid automobile insurance coverage Willingness to travel to homes, facilities, and community settings Preferred Qualifications Hospice or palliative care experience Clinical experience with grief counseling and end-of-life issues Knowledge of Medicare hospice benefit and Conditions of Participation Essential Skills and Competencies Strong clinical counseling and grief support skills Expertise in psychosocial assessment and crisis intervention Ability to guide patients and families through end-of-life challenges Excellent communication skills with emotional intelligence Cultural sensitivity and competence working with diverse populations Knowledge of community resources and social service systems Strong interdisciplinary collaboration abilities Understanding of family systems and dynamics Ability to maintain professional boundaries while providing empathetic support Deep commitment to hospice philosophy of care How We Care for You Minimum of 3 Weeks Paid Time Off (PTO) Medical, Dental, and Vision Insurance HSA and FSA options including Dependent Care Company paid Short Term Disability Company paid Life Insurance 401(k) with Employer Match Mileage Reimbursement Company Vehicle Program for field roles Pet Insurance ID and Fraud Protection And more...
