Job Description

The Clinical Social Worker provides social work services to patients and their families to help meet psychosocial and clinical needs. This role serves the needs of patients, both inside and outside the clinical setting utilizing therapeutic and other clinical techniques.
Essential Job Function:
Works with clinical staff to determine which patients to focus clinical social work efforts on to help staff and patients alike be in the correct environment with appropriate social work support. Collects information from both the patient and clinical staff to allow for ease of communication for all parties, limiting potential hiccups or gaps in information that could negatively alter the clinical experience. Connects patients to providers and community health opportunities that best support them to ensure that patients get the highest quality of necessary care. Develops a care plan for patients with the help of other clinical staff to enable the patient to get the treatment that is right for them. Educates staff, patients and families alike about critical information related to care to help all parties involved better understand the situation, serving as a liaison of sorts. Provides crisis support in high-tension situations for staff, family and patients using clinical expertise to ease all parties in high-tension situations, which do tend to arise in clinical environments. Performs other duties as assigned.
Education:
Required:
Master's of Social Work.
Experience:
Required:
Minimum of two (2) years of recent experience in a health-related environment.
Licensure/Certification/Listing:
Required:
Licensed Clinical Social Worker Associate.
Required:
BLS certification based on departmental/clinical setting or role assignment.
