A Healthcare Social Worker provides individuals and families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access to healthcare.

Job Description
- Provides social work services in accordance with the physician's plan of treatment.
- Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
- Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
- Serves as a liaison with other community resources. Job Requirements
- Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education .
- Currently licensed to practice as a social worker in the state of NC.
- Minimum of one-year Hospice experience preferred.
