Medi Home Health and Hospice , a proud member of the Medical Services of America, Inc. family, currently seeks an experienced Full-Time Medical Social Worker to join our Hospice team to cover Sparta (Allegheny County), NC areas . Responsibilities Include:
  • Provides social work services in accordance with the physician's plan of treatment.
  • Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
  • Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate.
  • Serves as a liaison with other community resources. Job Requirements
  • Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education.
  • Minimum of one-year Hospice experience preferred.
  • Valid Driver's License.
