The Hospice Medical Social Worker acts as a referral resource to outside agencies when the patient's plan of care warrants. What You'll Do As our Hospice Medical Social Worker, you will: Provide compassionate counseling and emotional support to patients, families, and loved ones throughout the hospice journey and during bereavement. Complete comprehensive psychosocial assessments and develop individualized care plans that address each patient's unique needs. Educate patients and families about hospice services, grief, loss, advance care planning, and available community resources. Collaborate with nurses, physicians, chaplains, and other interdisciplinary team members to provide holistic, patient-centered care. Assist families with care planning, living arrangement transitions, discharge planning, and connections to financial or community resources. Facilitate meaningful communication between patients, families, and the care team to ensure goals of care are understood and honored. Maintain accurate, timely documentation in accordance with agency policies and Medicare requirements. Participate in interdisciplinary team meetings, quality improvement initiatives, and educational opportunities. Support and mentor new social work staff and students as opportunities arise. Position Details Primary work location:

Requirements:
Masters of social work degree (MSW) from a school of social work accredited by the Council on Social Work Education One year of Social Work experience in a healthcare setting Interest in working with terminally ill patients and families
