• Full-time Hunterdon Hematology Oncology is a private medical practice that specializes in chemotherapy, infusions, and radiation treatments, as well as innovative treatments like immunotherapy and personalized targeted therapy to provide the best possible outcomes for our patients.

We are seeking an additional Licensed Clinical Social Worker to join our growing team. The Clinical Social Worker will report directly to the Executive Director and will be responsible for collaborating interdepartmentally to provide exceptional psychosocial care to oncology outpatients.

RESPONSIBILITIES

Performs initial and ongoing psychosocial assessments for oncology outpatients. Develops treatment plans in collaboration with patients, their support systems, and the interdisciplinary team, and communicates that plan to all team members. Provides limited supportive counseling to patients and families, and refers for long-term/in-depth counseling as needed. Provides crisis intervention to patients and families, and works collaboratively with the behavioral health team to provide safe and appropriate care. Refers patients and their loved ones to hospital-based and community agencies/resources, and provides assistance as needed to ensure optimal use of resources to meet patient needs. Provides assistance for transportation, financial management, and other resource acquisition. Serves as a link between inpatient and outpatient oncology, collaborating with inpatient and outpatient teams to ensure a seamless transition of care between settings. Advocates for and protects patients' rights, dignity, and confidentiality. Documents in the medical record according to regulatory and licensure requirements. In collaboration with the cancer care team, identifies opportunities for improvements in the care of cancer patients across the healthcare continuum, and actively participates in quality and performance improvement projects to motivate positive change. Collaborates with Hunterdon Healthcare and community agencies to identify barriers to care/resources and strategize actions to overcome those barriers for the oncology population. Collaborates with the marketing and Public Relations team to actively promote oncology psychosocial services, including but not limited to at least an annual review of all printed and electronic/web-based materials.

REQUIREMENTS

5+ years of prior experience as a Clinical Social Worker (required) LCSW (required) Prior experience in Oncology or Hematology (preferred) Strong written and verbal communication skills Familiarity with NextGen (preferred) Proficiency with Microsoft Office (Word, Excel) Proficiency with
