Setting:

Home Healthcare travel assignment Responsibilities Conduct psychosocial assessments for patients and families and contribute to care planning. Coordinate discharge planning and transitions to home-based services; collaborate with the clinical care team. Identify psychosocial barriers to care and connect patients with appropriate community resources and supports. Maintain accurate documentation of assessments, plans, and progress in patient records. Communicate effectively with clinicians, patients, and families; advocate for patient needs and safety. Ensure compliance with applicable regulations and standards; participate in care conferences and case reviews as required. Behavioral health experience will be rejected for this assignment. Requirements Licensed Social Worker in New Jersey (LSW) active and in good standing. Acute hospital experience required on MedSurg and ED units. Discharge planning experience.
