Monday-Friday, 9:00 AM-5:00 PM (Alternating weekends and holidays)

Hours:

75 hours per pay period

Pay Range:

$37.45-$56.17/hour Job Summary The Social Work Care Manager is responsible for conducting comprehensive psychosocial assessments and coordinating patient care throughout the hospitalization. This role identifies patients at high risk for readmission, develops individualized discharge plans, and collaborates with the interdisciplinary healthcare team to ensure safe, timely, and effective transitions of care. Key Responsibilities Complete comprehensive psychosocial assessments upon admission and reassess patient needs throughout the hospital stay. Identify high-risk patients using established screening tools and coordinate appropriate interventions. Develop and implement individualized discharge plans to ensure safe transitions of care. Coordinate multidisciplinary care within the hospital and with post-acute providers. Educate patients and caregivers regarding discharge plans, available resources, and follow-up care. Arrange post-discharge services, including community resources, home care, rehabilitation, and other supportive services. Collaborate with physicians, nurses, therapists, payers, and community agencies to optimize patient outcomes. Support departmental goals related to reducing length of stay (LOS), preventing readmissions, minimizing denials, and improving patient satisfaction. Maintain accurate and timely documentation in the Electronic Medical Record (EMR). Participate in quality improvement initiatives and perform other duties as assigned. Qualifications Master of Social Work (MSW) required. Active New York State Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) license required. NYS PRI/Screen Certification required within 90 days of hire . 1-3 years of experience in an acute care hospital or related healthcare setting preferred. Proficiency with Electronic Medical Records (EMR). Strong communication, care coordination, discharge planning, and interdisciplinary collaboration skills. #Allied2026
