Job Description
Job Summary We are seeking a qualified Clinical Social Worker for a hospice/medical social work contract position. The Social Worker will provide support and counseling to patients and families during the final stages of illness. Services may be provided in patients' homes, skilled nursing facilities, assisted living facilities, and adult foster homes. Responsibilities Provide medical social work services to hospice patients and families. Assess patients' psychosocial needs and develop appropriate care plans. Provide counseling, emotional support, and resources to patients and families. Coordinate with nurses, physicians, hospice aides, chaplains, and other healthcare professionals. Assist patients and families with end-of-life planning and available community resources. Document patient assessments, interventions, and follow-up appropriately. Support families with bereavement and other psychosocial needs. Travel to various patient locations as required. Requirements Oregon LMSW or CSWA license required. BLS required. Valid driver's license required. Reliable personal vehicle required. Auto liability/property damage insurance as required by law. Vehicle must be maintained in good working condition. Oregon LCSW. Hospital or medical social work experience preferred.
