Required Responsibilities Provide medical social work services to hospice patients and their families Support patients through the final stages of illness Provide psychosocial assessments, counseling, and emotional support Assist patients and families with care planning and community resources Collaborate with nurses, physicians, hospice aides, spiritual support, and other interdisciplinary team members Provide support in home settings, skilled nursing facilities, assisted living facilities, and adult foster homes Assist with bereavement and end-of-life support Requirements Minimum 1 Year of Social Work Experience Oregon LMSW or CSWA License Required BLS Certification Required Oregon LCSW preferred Previous hospice experience preferred Hospital or medical social work experience preferred Valid Driver's License required Personal vehicle required Auto liability/property damage insurance as required by law Reliable vehicle maintained in good working condition Preference will be given to candidates with previous hospice experience.
