$60.67/hrPosition Overview Clinical Social Worker to support patients and families receiving hospice services. The role provides medical social services, counseling, emotional support, and coordination of resources for patients and families during the final stages of illness. Hospice services may be provided in patients' homes, skilled nursing facilities, assisted living facilities, and adult foster homes across the service area. Requirements Oregon LMSW or CSWA license required BLS required Minimum 1 year of experience Hospital or medical social work experience preferred Previous hospice experience strongly preferred Oregon LCSW preferred Valid driver's license required Reliable personal vehicle required Auto liability/property damage insurance required • Interested?
