Cascade Health is seeking a compassionate Medical Social Worker to join our Hospice team in a resource role with no set hours each week . This position provides coverage based on department needs, including weekends, staff days off, and coverage at the Pete Moore Hospice House , as well as support for weekend home hospice admissions. You'll provide psychosocial support, counseling, care planning, crisis intervention, and community resources to patients and families at the Pete Moore Hospice House and in their homes.

This position will:

Provide counseling, emotional support, and crisis intervention to patients, families, and caregivers. Complete psychosocial assessments and develop individualized interventions. Participate as an active member of the hospice interdisciplinary team. Assist with caregiving plans, Medicaid navigation, and community resources. Initiate discharge planning when appropriate. Maintain timely and accurate clinical documentation. Participate in care conferences and interdisciplinary team meetings. Support students, interns, and new staff as appropriate. Provide coverage at the Pete Moore Hospice House and assist with weekend home hospice admissions. Qualifications & Requirements Master's degree in Social Work from a CSWE-accredited program. LCSW required; Oregon CSWA may be considered with completion of clinical licensure within three years of hire. Hospice or experience working with terminally ill patients and families preferred. Strong communication, counseling, assessment, and problem-solving skills. Ability to work independently and as part of an interdisciplinary team. Current driver's license, reliable insured vehicle, and insurable driving record required. Why Cascade Health?
