Medical Social Worker Job Description Physician's Choice is excited to announce a job opening for a compassionate and dedicated Medical Social Worker to join our dynamic team in a full-time capacity. This position is ideally suited for a skilled professional committed to making a positive impact in the healthcare field through direct patient engagement and community involvement. The candidate will have an opportunity to work closely with patients and medical professionals in home health settings, ensuring that all clients receive the best possible care tailored to their specific needs and circumstances. It is important to note that this is not a remote position. The role entails substantial in-person interactions as well as travel to patients' homes, complemented by phone call communication and administrative responsibilities to provide comprehensive support. Duties and Responsibilities Conduct assessments to determine patient needs and resources, including psychosocial, financial, and medical components. Develop and implement personalized care plans in collaboration with healthcare providers, patients, and families. Provide crisis intervention and support during medical or personal emergencies. Travel to see patients in their homes to offer face-to-face consultation and support. Conduct phone call visits to monitor patient progress and adjusting care plans accordingly. Coordinate with healthcare teams to ensure multidisciplinary care approaches. Advocate for patients' rights and needs within hospital systems and other care settings. Assist patients and their families in navigating the complex healthcare environment. Fulfill administrative duties related to patient care, including the documentation and confidentiality of medical records. Connect patients with local community resources and apply for these on behalf of the patient when necessary. Requirements Master's degree in Social Work from an accredited institution. Current valid driver's license and reliable transportation with valid car insurance as this position requires travel. Current Oregon medical social work license, valid and in good standing. CPR certification. Proven experience in a healthcare setting, preferably in home health or similar field. Strong communication and interpersonal skills to effectively interact with patients, families, and medical teams. Ability to work independently and as part of a diverse healthcare team. Experience with electronic health record (EHR) systems. Compassion and deep commitment to patient care with demonstrated empathy and understanding. Ability to manage multiple priorities simultaneously and maintain a high level of organization.
