Key Responsibilities Conduct thorough bio-psychosocial assessments tailored to the age and cultural background of clients, addressing mental health, substance abuse, and associated medical concerns within the primary care context. Provide direct behavioral health services, including individual and family counseling, initial intake assessments, diagnostic evaluations, treatment planning, and crisis intervention. Maintain detailed electronic health records by documenting evaluations, progress notes, and treatment plans in compliance with regulatory standards. Serve as a liaison between clients and community resources, facilitating access to local services such as medication assistance programs and support groups. Collaborate with our Quality Improvement teams to develop and implement effective strategies for data collection and client tracking to enhance service delivery and outcomes. Patient Population / Client Focus The clients you will serve represent a wide demographic within the Corona community, encompassing a range of ages, cultural backgrounds, and socioeconomic statuses. You will engage with individuals facing various challenges, including mental health disorders, substance use issues, and co-occurring medical conditions. Your role is vital in providing empathetic support and effective interventions that lead to improved health outcomes. Qualifications A valid Licensed Clinical Social Worker (LCSW) credential in the state of California is essential for this position. A Master's degree in Social Work (MSW) from an accredited institution, combined with a minimum of three years of post-licensure clinical experience. A strong working knowledge of counseling techniques, crisis intervention strategies, and independent judgment skills in clinical counseling are critical for this role. Preferred Experience Proficiency in conducting group therapy sessions and facilitating workshops geared toward mental health awareness and coping skills. Demonstrated ability to create and implement effective treatment plans for diverse client populations. Experience in collaboration with multidisciplinary teams to advance patient care quality and efficiency. Compensation and Benefits This position offers a competitive annual salary ranging from $115,000 to $130,000, commensurate with experience and education.
