The Medical Social Worker will collaborate closely with interdisciplinary teams to deliver holistic care that addresses both medical and emotional needs. Responsibilities Conduct thorough psychosocial assessments of patients and their families to identify needs, strengths, and barriers to care. Develop individualized care plans that incorporate behavioral health interventions, community resources, and support services. Provide clinical counseling, including child and family counseling, addiction counseling, and mental health therapy tailored to patient requirements. Assist with discharge planning by coordinating home care services, assisted living placements, or inpatient transitions while ensuring compliance with healthcare regulations such as HIPAA. Engage in crisis intervention and behavioral therapy for patients experiencing acute mental health or behavioral health issues. Collaborate with healthcare professionals on clinical research projects related to hospice care, dementia care, PTSD, or autism spectrum disorders. Maintain accurate medical documentation and ensure all records adhere to social work practice standards and legal guidelines. Participate in interdisciplinary team meetings to facilitate clinical interprofessional collaboration and optimize patient-centered care. Qualifications Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program; licensure or certification as required by state regulations. Proven experience working within hospital settings, hospice care, nursing homes, or outpatient clinics with a focus on medical social work. Familiarity with medical terminology, ICD coding systems (ICD-9/ICD-10), CPT coding for documentation purposes, and Medicare regulations. Strong knowledge of community resources including assisted living facilities, home health agencies, mental health services, and disability support programs. Experience working with diverse populations across cultural backgrounds; sensitivity to geriatrics, pediatrics, and individuals with disabilities. Proficiency in crisis management techniques such as crisis intervention and PTSD care strategies. Ability to perform patient assessments effectively while demonstrating excellent client communication skills. Knowledge of behavioral health therapies such as cognitive-behavioral therapy (CBT), motivational interviewing, applied behavior analysis (ABA), and group therapy modalities. Basic life support (BLS) certification including CPR training; experience in end-of-life care and hospice & palliative medicine is preferred but not mandatory.
