Job Summary We are seeking a dynamic and compassionate MSW to lead our community engagement efforts and support individuals through their healthcare journeys. Duties Develop, implement, and oversee volunteer programs that align with organizational goals, ensuring effective recruitment, training, and retention of volunteers. Conduct comprehensive client assessments to identify needs related to mental health, end-of-life care, addiction counseling, or developmental disabilities. Provide individual and group counseling sessions utilizing therapeutic techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and behavioral therapy. Coordinate with healthcare teams to facilitate discharge planning, home & community care, and hospice or palliative medicine services. Collect and analyze data related to client progress, program effectiveness, and community resource utilization to inform continuous improvement efforts. Maintain accurate medical documentation in compliance with HIPAA regulations and social work practice standards. Collaborate with community organizations to connect clients with resources such as assisted living facilities, special education programs, or senior care services. Participate in crisis intervention when necessary, offering immediate support for individuals experiencing mental health crises or trauma such as PTSD or dementia-related conditions. Experience Bachelor's degree in social work (BSW) or related field required; Master of Social Work (MSW) preferred. Proven experience working with diverse populations including geriatric patients, children, individuals with disabilities, or those receiving hospice & palliative care. Demonstrated knowledge of clinical research methodologies and medical terminology relevant to healthcare settings. Hands-on experience with EMR systems for documentation and case management purposes. Familiarity with medical records management, CPT coding, ICD coding (ICD-9/10), Medicare policies, and discharge planning processes. Experience working within inpatient settings such as hospitals or nursing homes is highly desirable; outpatient or home & community care experience is a plus. Strong understanding of social work practice standards, clinical ethical guidelines, HIPAA compliance, and crisis management protocols. Ability to work effectively with individuals from diverse cultural backgrounds while demonstrating empathy and professionalism in all interactions.
