This role assesses emotional, social, cultural, financial, and environmental factors impacting patient care and collaborates with the interdisciplinary team to develop individualized care plans that promote comfort, dignity, and quality of life. Key Responsibilities Assess the psychosocial, emotional, cultural, spiritual, and financial needs of hospice patients and families. Develop, implement, and evaluate individualized plans of care based on patient and family needs. Provide counseling, emotional support, crisis intervention, grief support, and coping strategies. Assist with advance care planning, end-of-life discussions, advance directives, and patient-centered goals of care. Educate patients and families regarding hospice services and available resources. Collaborate with physicians, nurses, chaplains, and other interdisciplinary team members to ensure coordinated patient care. Conduct social work evaluations and implement appropriate interventions. Connect patients and families with community resources, financial assistance programs, legal services, caregiver support, and respite care resources. Serve as an advocate and liaison between patients, families, caregivers, and community agencies. Maintain accurate and timely clinical documentation in accordance with regulatory and agency standards. Represent Mission Hospice professionally within the community and help foster relationships that support patient care and organizational growth. Qualifications Required Degree from an accredited School of Social Work. Current Utah Social Work license (SSW, CSW, MSW, or LCSW). Valid driver's license and reliable, insured transportation. Ability to travel within the service area. Strong communication, organizational, and interpersonal skills. Preferred Minimum one year of healthcare experience. Hospice, palliative care, home health, or related healthcare experience. Experience with counseling, grief support, and crisis intervention. CPR/BLS certification.
