This full-time position works Monday through Friday, 8:30 a.m - 5:00 p.m with flexibility to work later hours as needed.
Location:
Hybrid; Mostly Palm Beach and some Broward County Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness. What you'll
Do:
Qualified Social Worker responsible for assessing and intervening regarding the needs of chronically or terminally ill children and their families. The pediatric social worker is the primary social worker for the pediatric care programs including Hospice, PTFK, and Partners In Care. Provide emotional support, counseling, and crisis intervention to pediatric patients and their families, helping them navigate the challenges associated with serious illness, end-of-life care, and bereavement. Collaborate with an interdisciplinary team, including physicians, nurses, chaplains, and child life specialists, to develop and implement comprehensive care plans that address the psychosocial needs of patients and families. Connect families with community resources, financial assistance programs, and support services, while advocating for patient and family needs to promote optimal quality of life and continuity of care. Experience:
Master of Social Work (MSW) degree from a school of Social Work accredited by the Council on Social Work Education Preferably has a LCSW Requires 1 year of social work experience. Previous experience working with seriously ill children preferred. Must be comfortable working with pediatrics.
Continuing Education:
As required for licensure. For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion.
