Work Environment The position offers a unique opportunity to work in a fully remote setting, allowing you to manage your schedule while remaining focused on client care. This flexibility is complemented by structured work hours, Monday through Friday, from 9:00 AM to 5:00 PM. As part of our collaborative approach, you will engage with various stakeholders, ensuring that clients receive holistic and continuous support. Key Responsibilities Act as a liaison between clients, families, and outpatient providers, ensuring seamless communication and support throughout the care process. Intervene with insurance companies for pre-certification and utilization management, advocating for clients' necessary services. Contribute to discharge and aftercare planning by collaborating with clients to create individualized plans that promote ongoing recovery. Provide therapeutic meal support, offering guidance on building a healthy relationship with food while addressing mental health challenges related to nutrition. Deliver empathetic and affirming care for each client, recognizing and respecting their intersecting identities and circumstances. Patient Population / Client Focus In this role, you will work primarily with individuals in a detox setting. Our clients often face complex challenges related to substance use and mental health issues, necessitating a deep understanding of the dynamics of addiction and recovery. Your work will be pivotal in helping clients navigate their struggles and rebuild their lives. Qualifications A Master's degree in Social Work from an accredited institution is required. Professionals must possess a valid LCSW license in the state of Pennsylvania. Experience working with older adults is essential, as many of our clients are in this demographic.
