Our hospice center is equipped with the latest technology to support virtual therapy sessions, ensuring you can connect seamlessly with clients. Our collaborative team is composed of experienced mental health professionals who work together to offer integrated care tailored to the unique needs of patients. Key Responsibilities As a Licensed Clinical Social Worker, your day-to-day responsibilities will include: Conducting thorough clinical assessments to understand patients' mental health needs and develop tailored treatment plans. Providing therapy using various modalities such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) to address the unique challenges faced by patients. Maintaining meticulous clinical records in compliance with HIPAA regulations, ensuring confidentiality and security of patient information. Collaborating with a multidisciplinary team of healthcare providers to coordinate care and improve patient outcomes through shared insights and strategies. Participating in regular case consultations and peer reviews to enhance therapeutic approaches and share best practices. Educating patients and their families about mental health issues, treatment options, and available community resources to enhance their overall support network. Staying informed on the latest research and developments in the mental health field and integrating this knowledge into practice. Managing a full caseload effectively, ensuring timely progression towards established treatment goals while adhering to ethical guidelines set forth by the National Association of Social Workers (NASW). Patient Population / Client Focus Your role will specifically focus on patients receiving hospice care, a sensitive and critical area of practice. You will engage with individuals and families navigating complex emotional landscapes, providing support during end-of-life transitions. Your expertise will help clients cope with grief, loss, and existential concerns, fostering resilience and creating a nurturing space for healing. Qualifications The ideal candidate will possess the following qualifications: A Master's degree in Social Work from an accredited institution. Current and valid Licensed Clinical Social Worker (LCSW) license in the state of New York. A minimum of three years of experience providing clinical therapy, specifically in a mental health or hospice setting. Demonstrated knowledge of solution-focused therapeutic interventions and a commitment to ongoing professional development. Exceptional communication skills, with the ability to establish rapport and trust with diverse populations. Excellent analytical and problem-solving abilities to tailor interventions suit each patient's unique needs. Ability to work autonomously and as part of a cohesive treatment team. Preferred Experience Experience in hospice or palliative care is highly desirable but not mandatory. Familiarity with geriatric populations and the psychosocial aspects of terminal illness is beneficial in enhancing the quality of care provided to our patients. Compensation and Benefits We offer a competitive salary ranging from $80,000 to $100,000 annually, commensurate with experience.
