Job Description:
Duties/Responsibilities Manage a caseload comprised of medically and culturally diverse inpatient and outpatient families Assist families and patients by completing biopsychosocial assessments, identifying appropriate resources and referrals, assisting with discharge and transition planning, and serving as a liaison and an advocate between the patient and various systems Demonstrate knowledge of child development related to diagnosis, assessment, intervention, and referral of infants, toddlers, pre-school and school age children, and adolescents to appropriate community agencies Meet the needs of the families as identified, presented, or requested by the interdisciplinary team or physician Document information in the medical record consistent with departmental standards Maintain a working knowledge of a variety of referral sources and community resources appropriate for the patient population within the state of Texas Required Skills/Abilities Bachelor's degree or Master's degree in social work from an accredited (CSWE) Social Work Program required T exas State Board of Social Work Examiners (TSBSWE) licensure LBSW /LBSW-IPR or LMSW required Minimum 1 year of experience in health care social work preferred
