- Fort Worth is seeking a Case Manager to join our interdisciplinary team.
This full-time position will be a blended position supporting our Day Neuro clinic in addition to inpatient rehabilitation populations. The schedule for this position will be weekdays 8:00AM to 5:00PM with no weekend or evening hours. At our company, we support your career growth and personal well-being.

Responsibilities:
The ideal candidate will have experience in a medical or rehabilitation setting and be able to address not only clinical needs but also the psychosocial, functional and community reintegration factors that impact recovery and long-term outcomes for a complex neurologic patient population. The primary job functions in for the Case Management role include: Clinical Interventions/Discharge Planning Post-acute care planning management Payer/Referral Management Patient and family education and provision of community resources
Qualifications:
Minimum Qualifications Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline. Preferred Qualifications Previous experience in Case Management and Discharge Planning preferred. CCM Certification Preferred. #BSWIR•
