The hospice social worker provides emotional, psychosocial, and practical support to patients and their families facing end-of-life illnesses. The social worker collaborates with the hospice team to assess patient and family needs, coordinate community resources, provide counseling, and ensure patients receive quality compassionate care. Responsibilities Complete comprehensive psychological assessments for hospice patients and families. Develop and implement individualized care plans based on patient and family needs. Provide emotional support, grief counseling, and crisis intervention. Assist patients and families with advance care planning and end-of-life decision making. Educate patients and caregivers regarding available community resources and financial assistance programs. Coordinate referrals to community agencies and support services. Participate in interdisciplinary team (IDT) meetings and collaborate with physicians, nurses, chaplains, and other healthcare professionals. Maintain accurate, timely, and compliant documentation in the electronic medical record. Assist with discharge planning when appropriate. Support bereavement services as needed. Comply with all Medicare conditions of participation, state regulations, and agency policies. Qualifications Current Texas Licensed Master Social Worker (LMSW) in good standing. Master's degree in social worker from an accredited school of social work. Hospice, home health, palliative care, or healthcare experience preferred. Excellent communication, interpersonal, and organizational skills. Ability to work independently and collaboratively within an interdisciplinary team. Reliable transportation, valid driver's license, and current car insurance. Current CPR certification preferred.
