PRN Industry:
Hospice We are currently seeking an experienced LMSW (Licensed Master Social Worker) to join our hospice team on a PRN basis serving Tyler, TX and surrounding areas . The Medical Social Worker will provide social work services based on each patient's psychosocial assessment, as well as the individual needs of the patient and family. The Social Worker will work closely with the Interdisciplinary Group (IDG) to coordinate and support the patient's plan of care. Responsibilities Complete psychosocial assessments of patients and families. Provide social work services based on patient and family needs. Assist patients and families with coping, adjustment, and end-of-life concerns. Participate in coordination of care with the Interdisciplinary Group (IDG). Provide appropriate community resources and referrals when needed. Maintain accurate and timely clinical documentation. Collaborate with the hospice team to provide comprehensive, patient-centered care. Required Qualifications Master's Degree in Social Work (MSW) from a state-accredited program. Active Texas LMSW license required. Minimum of 1 year of hospice experience required. Previous experience as a Medical Social Worker preferred. Strong communication, interpersonal, and organizational skills. Ability to work independently and effectively as part of an interdisciplinary team. Why Join Us?
