Per Diem Position Overview We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our healthcare team in Arlington, VA. The ideal candidate will provide psychosocial assessments, counseling, crisis intervention, discharge planning, and community resource coordination for patients and their families. This role requires strong communication skills, the ability to work collaboratively with interdisciplinary teams, and a commitment to delivering patient-centered care. Responsibilities Conduct comprehensive psychosocial assessments of patients and families. Develop and implement individualized care and discharge plans. Provide crisis intervention and emotional support services. Coordinate referrals to community resources and support programs. Collaborate with physicians, nurses, case managers, and other healthcare professionals. Advocate for patients' social, emotional, and behavioral health needs. Document assessments, interventions, and outcomes accurately in the electronic medical record. Assist patients and families in navigating healthcare systems and available services. Ensure compliance with facility policies, professional standards, and regulatory requirements. Requirements Master's Degree in Social Work (MSW) from an accredited institution. Active Virginia Licensed Clinical Social Worker (LCSW) license. Previous clinical social work experience in a hospital or healthcare setting preferred. Strong knowledge of case management, discharge planning, and community resources. Excellent communication, organizational, and interpersonal skills.
