This position requires interaction with a variety of patient populations including, but not limited to, neonatal, pediatric, adolescent, adult and geriatric. The Emergency Department Clinical Social Worker 2 WIP position provides direct intervention with patients and families and jointly with the health care team, provides assessment, problem solving, advocacy, community referrals and crisis intervention services. This position is also responsible for the daily management of all complex discharge planning and social work functions. Licensure, Certification, or Registration Requirements for

N/A Independent action(s) required: Primary responsibilities include proactive development, implementation and ongoing revision of an interdisciplinary plan of care for patients who receive care in the Emergency Department. Negotiates and collaborates with the interdisciplinary team, community resources and payers to facilitate timely and cost-effective discharge plans that enhance appropriate health and social services in the continuum of care.
