As a Medical Social Worker with Care With Love, you will play a vital role in this team, offering thoughtful, one-on-one care in clients' homes, supporting each client's unique goals, and ensuring they feel truly seen, heard, and cared for. The Skilled Medical Social Worker:

Carries out social evaluations and implements a plan of care based on the evaluation findings. Provides social work services, including but not limited to short-term individual counseling, community resource planning, and crisis intervention. Assesses the psychosocial status related to the patient's illness and environment and communicates findings to the Clinical Nurse Manager. Serves as liaison between patients or families/caregivers and community agencies. Maintains appropriate clinical records and notes, and reports to the physician any changes in the patient's condition. Submits these records in accordance with Organization policy. Collaborates closely with a multidisciplinary team, physicians, and other wellness professionals to ensure a seamless and supportive care experience for every client. Uses compassionate communication to educate clients, caregivers, and families. Cultivates a client-centric environment of compassion, commitment, credibility, and cheerfulness—all core values at Care With Love. Qualifications Graduate of a bachelor's program in social work accredited by the Council on Social Work Education. May also have a master's degree or a doctoral degree in social work. One year of appropriate Home Health experience as an MSW preferred. Demonstrates good verbal and written communication and organization skills. Possesses and maintains current CPR certification. Can provide a copy of a negative TB test. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
